Not sure if anybody else has pointed this out already, but the ancient vision only spawns in if you attack a duplicate cultist while a phantasm dragon is alive which makes it just a little bit rarer

43:30 the noise.png is a pre-generated noise file, as generating noise is a pretty expensive operation. It is usually used during procedural world/texture generation where you want the same value to produce the same random value (like you can recreate seeds). for that it's common to use hashing, a function that produces very different outputs depending on the input, but is deterministic (e.g. hash(a) is always the same). for world generation a random value is invoked several times per tile and thus you either use a single thread (which is extremely slow), or you do them in parallel, and thus you need a stable algorithm, and in game design it's common to use pre-generated noise as it's much easier to read from a file than to generate it on the fly. (same reason most older games precomputed values for trigonometric functions)
