Lesson on Chronic Myeloid Leukemia (CML): Pathogenesis, Symptoms and Treatment. Chronic Myeloid (or Myelogenous) Leukemia (CML) is one type of leukemia, which is a blood cancer with high levels of white blood cells. CML is a leukemia with proliferation of mature neutrophils (PMNs), as evidenced by white blood cells with large nuclei on a blood smear or peripheral blood film. CML often presents asymptomatically, but can progress to having characteristic signs and symptoms of splenomegaly, constitutional symptoms, and lower sternum tenderness.

Pathogenesis of CML is due to the Philadelphia Chromosome, which is a product of translocation of chromosomes 9 and 22, leading to a new fusion gene and protein product. This new protein (BCR-ABL) is an activated tyrosine kinase, which leads to activation of cellular survival and proliferation pathways.

Treatment of CML involves a drug known as imatinib, which has been designed to specifically target the BCR-ABL fusion protein.

    Thanks for the video! I think another important point to mention for board exams is differentiating CML from leukemoid reaction using the leukocyte alkaline phosphatase test. The test will be normal to elevated in leukemoid react and decreased in CML.

  • i was diagnosed with cml around the first of june, 2022. they put me on bosilif 400mg daily. now my white count is normal and my red blood cells are starting to increase too. i go to my cancer dr every three weeks.

    I was diagnosed with CML in 2020 and my wbc has always stayed high. Yeah weeks ago I went and had my blood work done and I was at 247,000. I’m on Tasigna.
